Will there be co-pays, preferred doctors/hospitals a la HMO's, or medicaid-type reimbursements of doctors and hospitals - and if so will the reimbursements cover the actual cost of the care?

What about prescriptions and stuff like physical therapy?

What is there put in place for mental health patients - who are really undercared for in the current system...
