What motivation would there be for the better doctors to stay in the field? As hard as they work, as good as they get, they're compensated the same as the flunky down the street. There's zero incentive to attract and retain the best and brightest. Such a system cannot be defended even on the basis of "fairness" because it is fundamentally unfair to compensate the same for dissimilar deliverables.
