I'm a doc and I second Buynoski's diagnosis...all except for the infection thing-I doubt that. Infections of feet are common but not of hands.
Hand rashes are often difficult to treat. Medical terms eczema, contact dermatitis, nummular dermatitis are all terms to describe these difficult to treat rashes of hands and feet. Excess moisture definitely plays a role. In my area I see something similar to this in the feet of dairy workers who have to wear rubber or neoprene boots and their feet get wet inside and can't evaporate. My advice: try and keep hands dry more. Possibly a glove. Try an otc hydrocortizone cream occasionally when its bad, consider using gloves part time.
