I dislocated my left sholder twice in the same day. It took me several months to decide what to do but ended up having surgery. I also had a bone chip in the joint from the dislocation and would stick every one in a while but it needed to come out so I decided to have the torn rotater cuff and labrum fixed at the same time. The surgery was anything but fun and kept me out of boating for several months. I would agree on an MRI, rehab, and alternatives before you do surgery. I had three opinions, one specialized surgeon, a close friend of the family surgeon, and one DO. All recommended surgery because of the chip. However, I paddled  alot after the dislocation and before the surgery and felt great. All three of the opinions I got said they usually DON'T recommend surgery after one dislocation, but surgeons are trained to fix by surgery so I wouldn't be surprised if they recommend surgery, it doesn't mean it is the right thing to do, just like a doctor will most of the time recommend antibiotics for a cold or flu rather than letting your immune system fight it.  
On another note, I had a specialized surgeon that was very active in biking, running, swimming. As an athlete, he knew where I was comming from and we discussed all possible options. My sholder feels great to this day, no range of motion loss, arthritis, etc. Just wanted to share some of my experiences.
